In response to the COVID-19 pandemic, Assunta Hospital has introduced two value-added pharmacy services to ensure the safety and convenience of our patients, which are Pharmacy Park & Collect Service and Home Delivery for repeat medications.

Frequently Asked Questions (FAQ)

Yes, you will be eligible to enjoy these services regardless if you are self paying or if you are covered under a GL.

However, your eligibility to the services could be subject to additional restrictions/ conditions imposed by your insuring company upon your GL. For example, your GL might not cover for the medication delivery charges that may apply.

As a matter of fact, yes, you do need to have been issued a valid / legitimate prescription from Assunta Hospital by our doctors. And since the services are made specially for the refill of prescriptions (i.e. supplying of repeat medications for previously partial supplies), ‘fresh’ or ‘new’ prescriptions are not accepted at the moment.

Simple. All you need to do is click on the link above & you will be directed to an electronic sign-up/ request form. Fill in & upload the required documents.

Requests for the individual services should be submitted at least 7 working days before your previous supplies of medications are exhausted, or before the date you intend to collect/ receive (by delivery) your prescription refills.

We will reach out to you should your requests cannot be fulfilled due to certain reasons.

There could be a number of reasons as to why your requests for these services are unable to be fulfilled. Common ones being there are no valid prescriptions that have still pending refills, requests submitted / made too close to the requested collection / receiving date or, the terms & conditions applied onto the various GLs by the individual insuring entities. Another reason is that your prescription contains items which are not fit to be delivered via courier.

Certainly. You may get another person to collect your medications from our Park & Collect counter on your behalf. However, we will ask that your representative provide us your full name & complete identification number for the purpose of verification before we release the medications to your representative.

Yes, there may be a small fee charged for the delivery of medications to your doorstep. Please contact the Outpatient Pharmacy in Assunta Hospital for the latest rates.

Highly fragile items, temperature-sensitive products & other medications strictly regulated by Law such as psychotropic medications are all considered unfit for delivery.

Terms & Conditions

Please take note that by signing up for Assunta Hospital’s Park & Collect Service/ Medication Home Delivery service (referred to as “Services” hereafter), patient acknowledges that he/she has read, understands, and agrees to be bound by the following Terms and Conditions: -

  1. The Services are NOT for the selling/ supply of medications without valid prescriptions/ corresponding repeat slips (refill notes) from Assunta Hospital.

  2. The Services are available for the refilling of prescriptions issued by Assunta Hospital. Patients must provide a valid repeat slip (refill notes) during sign-up. New prescriptions are not currently eligible for these Services.

  3. Aside from the delivery charges, the Services are provided free of charge to patients who request the Services and meet the criteria set for the Services.

  4. The Services are rendered on a ‘per request’ basis, i.e. one (1) sign-up/request for one (1) round of refills or, in the case of home delivery, one (1) sign-up/request for one (1) delivery.

  5. Signing up for either/ both of the Services is only considered a request for the Services. All requests are subject to screening by the Pharmacy Department of Assunta Hospital (referred to as “Assunta Pharmacy” hereafter) against the criteria set for the Services.

  6. Assunta Pharmacy will contact patients who have put in their requests for the Services, within two (2) working days to notify them of the confirmation/declination of their requests for the Services.

  7. Requests may be declined due to, but not limited to, one or more of the following reasons:

    1. The request is submitted less than seven (7) working days from the selected collection date (Park & Collect)/ receiving date (Medication Home Delivery);

    2. Limitations or the Terms & Conditions set by the Insurance Companies or Third-Party Administrators (TPAs) upon the patient’s Guarantee Letter;

    3. For delivery of medications, there are items not suitable for delivery for example: dangerous drugs/psychotropic medications; and

    4. Patient with a past record of being uncooperative and/or verbally abusive during previous provision of the Services.

  8. Assunta Pharmacy requires a minimum of seven (7) working days from the selected collection date (Park & Collect)/ receiving date (Medication Home Delivery).

  9. Assunta Pharmacy shall not be held liable for any delay in the Services in the following situations: -

    1. Where the request is submitted less than seven (7) working days from the selected collection date (Park & Collect)/ receiving date (Medication Home Delivery);

    2. Where there is a delay on the patient’s end in the submission of required documents (e.g. prescription, repeat slip, etc.);

    3. Where there is a delay by the patient in responding to Assunta Pharmacy’s correspondence requesting additional information/ confirmation of information;

    4. Where there is a delay in payment (self-paying patients);

    5. Where the delay is caused by the Delivery Service Providers in delivering the medication; and

    6. Which caused by unforeseen circumstances beyond our control, including but not limited to natural disasters, pandemics, governmental actions, and/or strikes. In such events, we will notify patients as soon as possible and provide alternative solutions where feasible.

  10. All decisions made by Assunta Pharmacy to accept or decline requests for the Services are deemed final. However, patients may appeal within seven (7) days from the date of receiving the decision by contacting Assunta Pharmacy. Each appeal will be reviewed on a case-by-case basis.

  11. Assunta Pharmacy reserves the right to withhold/ cease/ terminate the provision of any of the Services to patients who have signed up in, but not limited to, the following situations: -

    1. There is duplication of supply/ overlapping prescriptions;

    2. There is/ are overlapping prescription(s) or newer prescription(s) that supersede(s) earlier prescription(s);

    3. There is/ are error(s) to the information entered by the patient during the sign-up/ request for the Services;

    4. There is/ are change(s) to the sign-up information leading to a discrepancy between the actual & the recorded information;

    5. Patient fail to cooperate at any point in/ along the process of rendering him/ her any of the Service(s); and

    6. Patient is deceased.

  12. More on Medication Home Delivery Service: -

    1. Assunta Pharmacy ensures that all items are in good condition when the items are handed over to the Delivery Service Providers. However, once the items are with the Delivery Service Providers, the risk of damage or loss transfers to the patient;

    2. Assunta Pharmacy shall not be responsible for any non-deliveries resulting from incorrect addresses provided during the signing up for the Services;

    3. Once deliveries are in transit, patients will not be permitted to cancel their requests; and

    4. In the event of any issues with the deliveries, patients should contact Assunta Pharmacy within 24 hours of receiving the package or 72 hours of receiving the tracking number from Assunta Pharmacy (for non-deliveries) to facilitate resolution with the Delivery Service Providers.

  13. There shall be no claims by the patient for any losses, thefts or damages against Assunta Pharmacy once the items have been collected (Park & Collect Service) or accepted (Medication Home Delivery Service).

    1. All personal information collected during the sign-up/request process is used solely for providing the Services. Assunta Pharmacy adheres to Malaysia’s Personal Data Protection Act (PDPA) 2010.

    2. We implement strict security measures to protect your data. Personal information is stored securely and is accessible only to authorised personnel.

  14. Assunta Pharmacy & Assunta Hospital reserve the right to amend any of the Terms & Conditions herein listed in part or in full as deemed necessary without prior notice.
